Eligibility and Requirements
To reserve or operate a COSE vehicle, the driver must:
- Hold a current, valid California driver’s license
- Have a current Defensive Driver Training certificate
- Complete an additional SFSU Van Safety Training (via CSU Learn) if driving a vehicle that accommodates ten (10) or more passengers

Reservations
Reservations for COSE vehicles should be made using the link below.
Before submitting a request, review the COSE Vehicle Use Policy and all associated charges.
- Have your account number and funding source ready.
- A $3.00 per mile usage charge applies to all vehicle trips.

Pre-Trip and Post-Trip Requirements
Drivers must complete the following forms before and after each trip:
Printed QR codes for these forms are available inside each vehicle folder.
Safety Equipment
Each COSE vehicle includes:
- A fire extinguisher
- A first aid kit
Additional items available by request when reserving:
- Tire inflator / portable air compressor
- Portable car jump starter (power bank jumper)
Van Safety Training Requirement
For vehicles that accommodate ten (10) or more passengers, drivers must complete the SFSU Van Safety Training course through CSU Learn.
This training takes less than 15 minutes.

COSE Vehicle Use Policy
- The driver must have a current Defensive Driver Training certificate.
An additional SFSU Van Driver Training certificate is required only if the vehicle holds ten (10) or more passengers. NO EXCEPTIONS. - The driver has not been issued more than three (3) moving violations, nor been responsible for more than three (3) accidents, or any combination thereof within the past twelve (12) months.
- Faculty on FERP may drive COSE vans only during their active teaching semester(s). Any vehicle use will be charged back to the respective faculty or department.
- Transporting or consuming alcoholic beverages in any state-owned vehicle is strictly prohibited (zero tolerance). No smoking in the vehicle.
(This restriction does not apply to laboratory-use alcohol or other scientific-grade chemicals transported under University safety procedures. Transport of hazardous chemicals must follow the SFSU EH&S Moving Chemicals SOP, which requires secondary containment, trained personnel, and safe handling measures.) - Return the vehicle with a full tank of gas. Remove all trash and debris. Failure to clean up may incur a cleaning fee.
- Keys: Pick up and return at the Biology Stockroom (HH 539) — check opening hours beforehand.
- Perform online pre-trip and post-trip inspections. Complete the checklist and report any issues immediately to COSE vehicle management.
- In case of an accident: Take photos, complete STD 270 (form in car folder), and notify COSE (Vincent Cheung, vincentcheung@sfsu.edu). Submit STD 270 to your supervisor and notify SF State Enterprise Risk Management at (415) 338-2565 or riskmgmt@sfsu.edu. If police were involved, include the report number with STD 270.
